Medical Requirements

You will be required to complete a Medical questionnaire and, before diving in open water, sign a self-certification that you are fit to dive. You should be totally honest and to sign the declaration of fitness only if you are totally happy you fulfil all the requirements. In turn, the Proteus SAC (Southwell) undertakes to respect the confidentiality of your documents absolutely, and to reveal the contents only to correctly qualified and authorised persons.
